Athletic Forms

All forms/physicals must be dated after May 1. The following documents and forms should be downloaded, reviewed by a parent/guardian, and uploaded online here:

*Emailed physicals will not be accepted and Coaches will not take them in person. They MUST be uploaded to the OC Student Athlete Portal!

Return-To-Play 

Clearances should be given to the Athletic Trainer directly. *The School Office will not accept dropped-off or emailed physicals. Coaches will not take them in-person. 

 

 

Athletic Training Services

UPMC Sports Medicine is the proud provider of Athletic Training Services for Oakland Catholic High School.

 

Sports Physical Procedures

In accordance with PIAA rules and regulations, no student athlete will be allowed to participate in ANY athletic activity until they have submitted a completed sports physical and consent to treat (PIAA PPE & UPMC HIPAA) to the Oakland Catholic Athletic Department via Final Forms.

DO NOT submit physicals to the School Office, School Nurse, Coaches, Athletic Director, etc.
Paper submissions are NO LONGER ACCEPTED.
Please make a copy for your records prior to uploading any paperwork.

  • Fall Sports: Student Athletes must have the PIAA PPE and UPMC HIPAA document submitted to Final Forms by Monday, August 15th, 2022 in order to participate in the fall sports season.
     
  • Winter Sports: Student Athletes must have the PIAA PPE and UPMC HIPAA document submitted to Final Forms by Monday, November 21, 2022 in order to participate in the winter sports season. A student athlete that participated in Fall Sports must ALSO submit to Final Forms a “"Parent Re-Certification" form (Section 8).
     
  • Spring Sports: Student Athletes must have the PIAA PPE and UPMC HIPAA document submitted to Final Forms by Monday, February 27, 2023 in order to participate in the spring sports season. A student athlete that participated in Fall or Winter Sports must ALSO submit to Final Forms a “"Parent Re-Certification" form (Section 8).

 

UPMC provides a certified Athletic Trainer to aide in the prevention, recognition, evaluation, and treatment of athletic injuries. The Athletic Trainer is also responsible for on-site first-aid and emergency management at games and practices for at risk sports at Oakland Catholic.

If your student-athlete is injured or has an illness, encourage them to be seen by the Athletic Trainer for further evaluation. Upon evaluation of the student athlete, the Athletic Trainer may:

  • Provide the athlete with exercises to be performed at home
  • Offer rehabilitation in the athletic training room
  • Suggest a taping or bracing technique to assist the athlete
  • Recommend limited or no practice
  • Refer to a physician for further evaluation

No matter what the plan may be, the athlete should be encouraged to follow up with the Athletic Trainer as necessary.
